Allies Voice: Performance enhancement or damaged goods?
ESPN calls today's Capitol Hill hearing on HGH use in Major League Baseball "Damaged Goods", but I call it resoundingly familiar in the fairytale I know as "diabetes control". These guys were just trying to obtain physical excellence any way they could obtain it to put a smile on the fans faces and please their coaches. Controlling diabetes is really no different - especially nowadays when everything is performance enhanced. Looks like major league baseball isn't the only legacy with damaged goods.
Once upon a time insulin therapy was a lot easier. The job of managing diabetes was actually easier because you didn't have all of the "performance enhancement" options - satellite glucose monitoring, unleaded premium patented Franken insulins, and infrared frequency controlled pumps. This is because we're all trying to impress our doctors with nothing but homerun numbers.
How is this any different than professional athletes looking for "performance enhancements"? The way I see it - athletes are only doing what their coaches tell them to do to keep their bosses happy, the fans happy - and win games. Is this any different than a patient with diabetes taking what their doctor says is the best for their glucose performance? We want to keep our doctors happy and win the game too, right?
Back in the day (circa 1983 and before) you could bet in a tight range your insulin efficacy. You also had confidence in knowing your insulin would be around for more than just a few years. In recent testimony before Congress discussing generic biotech pharmaceuticals, a Novo Nordisk representative stated they will eventually move on to all analog "insulins." If you take a look at history, based on this statement, consider the statement that in 1993-94, Beef Ultralente was removed from the marketplace and the removal of ALL natural insulins took place over the ensuing decade. Then also based on history, Human Ultralente (which was the replacement for Beef Ultralente) was also removed from the marketplace during this timeframe.
If you are new to insulin - as in the last 5 years or so - you'll start to notice a fleeting trend when you cozy up to insulin you think works best for you. Newer drugs are always in the pipeline at the pharmaceutical companies. It's a shame, but the only thing that has remained consistent in the drug market of diabetes is the life-saving glucagon shot. Life-saving if it's not the last $100 in your pocket. A glucagon kit costs anywhere from $75 to $250! I want to thank Bernard Farrell for bringing that ugly truth to my attention. Why do you have to pay so much for something that should be feasibly available, if not partnered with the danger itself?
Here's a riddle for the thinkers: Now we are coming to that period of time where the processes for making Human Insulin (Humulin, Humulin NPH) will be losing patent protection. We must realize that it is the PROCESS--not the product-that is afforded patent protection. Biotech companies are placing pressure on the FDA/government to allow production of this "common" hormone. Acting as obstructionists, the current insulin cartel is imploring the FDA to REQUIRE bioequivalence testing for generic manufacturers, disregarding the fact that they, themselves, have never been held to this standard. How does one resolve such voodoo economics? Anybody? Anybody? Bueller? Bueller?
Brent Hoadley, the author of "Too Profitable to Cure"", had a few ideas as to where Big Pharmaceutical might run interference and their usual shenanigans (yes, I love italics). Some might also call it diabusiness - either way, it affects us all.
Who's to say Novo and Lilly won't decide to take their ball and go home. This leaves the FDA with nothing to use for bioequivalence of the product or chemical identity to the product no longer in the marketplace. At this point, biotech companies have no other option but to work on and patent new insulin-like compounds. Unless, of course, the best diabetes drugs of yesteryear were actually NOT drugs at all...but natural insulins. Last time I checked the polypeptide sequence of bovine or porcine (beef or pork) insulin and the concentrations of zinc or protamine for Regular or NPH - it looked the same as it ever was! :::Queue the music:::

Your Glucagon analogy reminded me of those Bee Sting Kits for an Adrenaline Shot - again $75 Bucks!! Who the Heck besides Fat Pharmaceutical Execs can afford that! I am Allergic to Mustard + Dill - which fortunately I can Avoid but if yer Eating out in a Resto - Mustard is buried in Everything to Zip it up!
Also fortunate is having Benadryl + if taken Early enuf it will do the Trick but sometimes U do need an Epinephrine Shot to clear it out Immediately - especially Folks allergic to Peanuts or ShellFish.

We have already reached the point where the processes for making Human Insulin (Humulin, Humulin NPH) have lost patent protection. For Humulin R and Humulin N, the patents expired in 2001. For Novolin R and Novolin N, those patents expired in 2002, so anyone can now copy the processes used to make them.
Also, insulin being grandfathered under the Federal Food, Drug & Cosmetic Act (not the Public Health Services Act which governs most other biopharmaceuticals) means there is no legal impediment to generics RIGHT NOW. But the FDA has refused to outline procedures for generics manufacturers to get approvals. The law isn't holding this up, it is policy at the FDA.

Well I just placed an order in for well the insulin that is keeping me alive and believe me there is no other option for me and I refuse to inject anything else into my body other than animal insulin but guess what here is the cost so far, $25.00 dollars to mail the order to England, $4,280 dollars US because the pound today is 1.97 to every dollar not to mention the extra $60.00 dollars to have TNT deliver it to Customs or or the FDA and well that will take a couple of weeks for sure so let's see know for a 6 month supply of insulin that is keeping me alive is $4,400 plus or minus a few dollars. This is place on my credit card which will be maxed out shortly just to get the very medication or hormone or what ever you want to call it. For it's all or nothing. Please tell me any other medical condition that requires so much paper work, hassle, stress, and expense just to live a somewhat normal life. I just want to thank Eli Lilly and Novo and all the rest who failed the Diabetic population the option of choosing what is good for them. Thank you for all your help. :0

have you heard of an insulin still with c-peptide still intact? Any bovine or porcine insulin produced somewhere outside USA?

Glenn--My last order had an additional $60 cost to get TNT to deliver the product to my door. (The first $60 TNT charge was noted on my invoice; the second $60 charge occurred when I called to find out when my shipment would be cleared through customs and shipped to me. I was informed there was an additional $60 delivery charge--and required to supply credit card info to complete the transaction. In almost any other area, this would be called EXTORTION.)
And you have failed to explain the stress caused by delivery uncertainty: (1) will the shipment survive high-altitude transit (one of my shipments had four bottomless, empty vials); (2) will shipment be stored at proper temperature; (3) will customs clearance go smoothly--or will this be the shipment where Dept. of Homeland Security steps in and confiscates my "illegal" drug--BECAUSE THEY CAN?

In response, to Glenn V and Melody, I am so disgusted to hear how difficult it is for you to get your insulin. Is there anyone out there that can help? Allie, can you help them? It is not only vital that they receive their insulin, but that it is not damaged because of temperature changes, etc.
My heart goes out to both of you. I will be praying and placing a call to heaven. Again, I am so sorry to hear this.

Apparently, it doesn't look TOO hard to get porcine insulin in Canada, as long as this statement below is current. Perhaps it would be easier for some Americans to order their insulin from here instead of the UK?

It's easy to get porcine insulin through Canada; they've been very helpful to me. You can get it directly online without a doctor's prescription. IDDT.org says you can drive over the border to physically buy it; but, I haven't had luck yet in finding it.
To make an online purchase, you can go to http://www.getcanadiandrugs.com. When you sign up for a user's account, they require you to sign that you've used the drug before. I haven't used it, although I do have a prescription so I called the company to ask what to do. They told me to click "I agree" anyway.
Through getcanadiandrugs.com, it costs $131.99 U.S. per vial plus (for me at least) $14.99 U.S. in express "cold" shipping. They say on the website it takes 8-14 days, but when I called them, they told me it's closer to 3-8 days depending how long it gets delayed in customs.
Through the manufacturer, Wockhardt, you can get each vial for $25 pounds (or $49.17 U.S.) but that excludes the USDA permit ($94 U.S. for a 1 year permit, $61 U.S. per year to renew), administration costs ($98 U.S. per order) and shipping costs ($114 U.S. per order) which for 6 vials adds up to $1,085 U.S., which equates to $180/vial at a minimum, so it's a lot cheaper to go through getcanadiandrugs.com.
Another consideration is, with Wockhardt, you have to jump through the hoops (there are 5), including getting a physician to write a letter of medical necessity and sign Wockhardt's "Compassionate Use" form (which is anything but compassionate and will need an understanding physician to sign).
I've opted to go through Canada.

Diabetic nephropathy (DN) is one of the main causes of KIDNEY disease and a leading cause of diabetes mellitus related morbidity and mortality.
Recently, SIRTUIN are reported to have emerging pathogenetic roles in cancer, muscle differentiation, heart failure, neurodegeneration, diabetes and aging.
The aim of the present study was to study the role of intermittent fasting (IF) on DN and studying the expression of SIRTUIN ... Sir2 and p53.
At biochemical level, it was found that IF causes significant improvement in blood urea nitrogen (BUN), creatinine, albumin and HDL cholesterol, parameters that are sometimes associated with the development of DN. There parameters suggest that DN may be in-progress but it is other than clear which of the parameters are part of a healing process helping to combat developing DN.
Diabetic rats on IF also show significant, apparently beneficial, changes onset of hypertension.
Interestingly, the expression of Sir2, a NAD dependent histone deacetylase, decreases in diabetic rat kidney and this decrease is overcome by IF.
Evidence is also provided for involvement of mitogen activated protein kinases (MAPK) cascade in mediating the effects of IF as there is reduction in the expression of p38 which gets induced under diabetic condition. This was further accompanied by a decrease in p53 expression.
These findings suggest that IF, by way of a 'One Meal every day' (SUB15 minutes) intermittent-fasting program, should slow, halt or reverse the development of Diabetic neuropathy...'].

One performance enhancement is primarily about the money, and secondarily about the history. Of course a player wants in the hall of fame, but the primary concern is to get set up in the hear, now, and foreseeable future. The hall of fame is primarily for one's children, grand-children, great-grand-children....
